Incident Overview

On November 4, UPS Flight 2976, which was destined for Hawaii, crashed shortly after takeoff from Louisville International Airport, the global aviation hub for UPS. The aircraft, a McDonnell Douglas MD-11F, failed to gain altitude and came down in a commercial area, impacting several businesses.

Casualties and Injuries

The crash resulted in the loss of all three pilots aboard, along with 12 individuals on the ground, including Colina. Nearly two dozen others sustained injuries during the incident. The aircraft was carrying approximately 20,000 packages and 38,000 gallons of fuel at the time of the crash.

Preliminary Investigation Findings

Following the accident, the National Transportation Safety Board (NTSB) began an investigation. According to their preliminary report, the plane reached an altitude of just 30 feet before descending. Notably, video footage showed the left engine separating from the wing during takeoff, and evidence of cracks was discovered in the left wing’s engine mount.

Todd Inman from the NTSB stated that the debris field from the crash extended for half a mile. Moreover, a cockpit vostart recorder, start of the plane’s two black boxes, captured a persistent alert sound in the cockpit for approximately 25 seconds as the aircraft crashed.

Ongoing Investigation

As the NTSB continues its investigation, they anticipate that a comprehensive final report may take up to two years to complete. The details released thus far underscore the complexity of the incident and highlight the ongoing efforts to understand the causes behind this catastrophic event.
